International Student Health Insurance Information
International students with an F or J visa who are registered at Fort Hays State University are required to have proof of adequate health insurance coverage as mandated by the Kansas Board of Regents and immigration regulations.
All international students with an F or J visa who are registered in courses at FHSU will automatically be enrolled in the UnitedHealthcare StudentResources insurance plan.
Fees for the UnitedHealthCare StudentResources insurance plan will be automatically charged to the students account and any payments made will be applied to the health insurance charges first.
COST FOR HEALTH INSURANCE:

Fall 2024 semester is $1,179.50 and coverage is August 1 - December 31, 2024.

Spring/Summer 2025 semester is $1,651.50 and coverage is January 1 - July 31, 2025.
Dependents (spouse and/or children) accompanying the student to the U.S. on an F or J visa are required to also have proof of adequate health insurance coverage.

Understanding Health Insurance in the U.S.
If international students choose to provide comparable qualifying health insurance coverage from another provider and want to waive the UnitedHealthCare StudentResources insurance plan, students will need to go through the waiver approval process. Students will need to take the following action:
- Complete the Student Health Insurance Waiver Form
- Complete the Student Health Insurance Acceptance/Denial Form
- Provide copy of health insurance summary of benefits in English
- Provide copy of health insurance card or certificate
- Provide U.S. health insurance provider contact information--phone number & address
The documents listed above must be received in the International Student Services Office no later than the first day of class.
International students who have provided proof that they have comparable health insurance coverage may request a waiver to this automatic registration of the school’s health insurance plan by submitting the waiver forms.
For international students "comparable health insurance coverage" means that your health insurance plan meets all the waiver criteria on the Student Health Insurance Waiver Form.
- Coverage for essential benefits (with no dollar limits) is defined under the Patient Protection and Affordable Care Act (ACA). Stand-alone travel and/or emergency/urgent care coverage is not acceptable. The policy must include:
- Pharmacy
- Mental health services
- Maternity benefits
- Preventive care
- Coverage for pre-existing conditions
- Pediatric dental and vision coverage (if you have dependent children on the plan)
- Unlimited Maximum Benefit for covered medical expenses.
- A policy year deductible of $500 or less.
- Maximum total out-of-pocket expenses cannot exceed $6,350 per member ($12,700 per family) with preferred providers (deductibles, coinsurance, and copays all count toward the out-of-pocket maximum).
- A minimum of 80% coinsurance payable by the insurance plan to network providers.
- Coverage includes effective dates spanning the entire period for which the waiver is requested.
- Plan document(s) are written in English, with currency amounts converted to U.S. dollars, and the insurance company contact phone number is in the U.S.
- Insurer has a base of operations in the U.S. or has a U.S. based claims payer.
- Policy must be underwritten by an insurance company that meets the rating requirements of the U.S. Department of State Exchange Visitor Program or is backed by the full faith and credit of your home government.
- Insurer is authorized to do business in Kansas and is providing coverage under a policy that has been filed and approved by the Kansas Department of Insurance.
- At least $100,000 in coverage for repatriation and medical evacuation benefits.
