Scheduling an Appointment
The Kelly Center provides personal counseling, drug and alcohol counseling, and accessibility services to current FHSU students, faculty, and staff. To schedule an appointment, call our office at 785-628-4401.
Patient Portal
Our Patient Portal provides easy access for completing forms that will help us learn more about you and your needs. It is important that you complete all required forms prior to your appointment. You must first call our office to discuss services BEFORE completing any forms in the Patient Portal.
From the Patient Portal you can:
- See all of your upcoming appointments.
- Complete required forms and route them to your provider.
- Complete Triage informational forms if you have been instructed to do so to start receiving services.
- Complete a Release of Information form to other individuals, providers, or agencies. (A maximum of 1 release request per day can be completed.)
Accessing the Patient Portal
To access the Patient Portal, you will use your TigerNetID and password. If you are a student, your TigerNetID account is created automatically when your college admission's application is processed. This is the same account you use to access your university Gmail account, TigerTracks, Blackboard, TigerCentral, and other university resources.
