Notice of Accessibility
Fort Hays State University will ensure that no qualified person with a disability is denied the benefits of, excluded from participation in, or otherwise subjected to discrimination because of inaccessibility to employment, education programs and all activities of Fort Hays State University. Process for Seeking Accommodations
Fort Hays State University acknowledges its responsibilities under Section 504 of the Rehabilitation Act and the Americans with Disabilities Act. Those obligations include providing reasonable accommodations to otherwise qualified persons who have a disability as defined by the ADA.
It is the responsibility of persons with a disability to notify the University of that disability and the need for accommodations. Assistance from qualified experts may be necessary to determine whether an accommodation requested is reasonable.
To register, please submit the Faculty & Staff Reasonable Accommodation (ADA) Request Form. Employees will also be required to have a qualified healthcare practitioner complete the Medical Inquiry Form. These documents will be reviewed to determine if an employee meets the definition of an individual with a disability and to identify potential accommodations.
