Academic Accommodations
To receive academic accommodations for the first time at FHSU, you must register with Student Accessibility Services by submitting documentation and completing the three-step process outlined below.
The Three-Step Process
- Step 1: Complete and submit the Academic Accommodations Questionnaire.
- Step 2: Submit documentation of your disability. See documentation guidelines below.
- Step 3: Student Accessibility Services will review your documentation and contact you when it is time to schedule your intake appointment. This appointment may occur in-person or by phone/video. Documentation review can take 1-2 weeks.
Once Steps 1 & 2 have been completed, you will be notified by the Accessibility Services Advisor regarding the decision. If academic accommodations are approved, the decision is provisional pending completion of Step 3. Step 3 should be completed during the first few weeks of the semester or over the summer. Please note, if additional documentation/information is needed, you will be contacted by the Accessibility Services Advisor.
Review of documentation typically takes 1-2 weeks; however, this time may be longer during high volume months.

Documentation Guidelines
If you have a disability that substantially limits one or more major life activities, you may be eligible for academic accommodations. To determine eligibility, we will need comprehensive and relevant documentation from an appropriate, licensed healthcare professional that explains the impact of your disability and makes a recommendation for accommodation.

Students Transitioning from High School to College
High school is very different from college, especially when talking about differences in accessibility services. We have developed a transition guide to help you understand many of these differences. The guide includes insight on the differences in who initiates accommodations, parental involvement in the accommodation process, and important confidentiality information. To ensure that accommodations are in place when you come to college, it is important that you initiate the three step process outlined above as soon as you have registered for classes at FHSU.
