Syllabus Template
The syllabus template includes the essential elements to develop a quality course syllabus. The style features, such as colored headings, graphics, pictures, links, etc., can be adjusted to meet your course, department, or college needs.

General Education Courses
Use the following resources to download the templates and guidelines needed to submit a General Education Course, then submit the appropriate course changes form below.
New Course Approval
All course proposals and changes will be submitted to curriculum@fhsu.edu.
Please download a copy of the form to prepare your syllabus and make a note of the submission questions to be answered on the form. Also, review the Course Development Guidance to learn more about the expectations and requirements.
A New Course Proposal is also required for:
- converting variable title courses to permanent courses
- changing an undergraduate course to a 600/600G course
Please complete the New Course Proposal Form when you are ready to submit a course.
Existing Course Changes
Please submit the Minor Course Changes form if:
- Minor Title Change
- Course Number Change within the same level (e.g., 201 to 221)
- Pre- or Co-Requisite Change
- Minor Course Description Change
- Reduction in Credit Hours
Please submit the Significant Course Changes form if:
- Course Number Change to a different level (CLO changes should accompany this)
- Change in Course Learning Outcomes
- Aligning a Course to General Education
- Increase in Credit Hours (CLO changes should accompany this)
- Significant Title Change
- Significant change in Course Description
- Retiring a Course
