Program Approval
All program proposals and changes will be submitted to curriculum@fhsu.edu.
- New Degree Program Template (KBOR)
A new Degree Program must be sent to KBOR and HLC for approval after receiving approval from all levels at FHSU. Prepare the New Degree Program Template for this request to accompany the New Degree Proposal form for approval routing.
- New Degree Program Proposal (Internal)
This form is for submitting a proposal for a new degree program. - Program Curriculum Map
The Program Curriculum Map is required in addition to the proposal template. - New Concentration Program Proposal
A new Concentration Program is developed based on an existing degree program and only requires FHSU approval. Prepare the New Concentration Program Template for this request.
- New Minor or Certificate Proposal
Minors and Certificates are requested on the New Minor or Certificate Proposal form.
Program Change
Please use the appropriate link below to submit a Program Change.
A minor program change includes aggregate changes to approved degrees (including concentrations) totaling less than 25% of the program since the last HLC accreditation review (comprehensive evaluation or Assurance Review).
- Coursework changes (such as swapping one course for another of the same level)
- CIP Code change
- Shifting the same number of Credit Hours between categories (such as adding a course to a Concentration and reducing one from Electives)
Significant program changes are aggregate changes within the approved degree (including concentrations) totaling greater than 25% of the program since the most recent HLC accreditation review (comprehensive evaluation or Assurance Review).
- Changes to GenEd within a major
- Addition of program pre/co-requisites
- Addition or reduction of total program Credit Hours
- Changes to Program Learning Outcomes
- Changes to required clinical experiences
- Changing a program's modality
